DALN Forms (print)

If you send a narrative to the DALN by post, it must be accompanied by all four of the following forms (signed and completed) before we can post it on DALN. If you send multiple narratives, each must be accompanied by these four forms.


Form #1: Informed Consent
Because we value your right to make an informed decision to participate in the DALN, we must have your consent before we accept a submission. If you are over 18 years old and consent to participate, please choose "Adult." If you are under 18 years old, your parent or legal guardian must consent for you by choosing the "Under-18" option.

Form #2: Release Form
This form tells you what information the DALN makes public when it posts your literacy narrative. Form #3 Deed of Gift OR Creative Commons License
The Deed of Gift form will allow you to transfer the rights to your literacy narrative to the DALN.

If you send in your contribution by post, this form will provide us the information we need so that users can search for your narrative and find it.
