Submitting Your Literacy Narrative by Mail

You can submit your literacy narrative by mailing it to us, but all the parts (documents, photographs, music, images) must be in a digital format. To submit your literacy narrative by mail, follow these steps:

Step 1: Tell your story and collect the items you want to include.
Step 2: Digitize the various parts of your story. And burn these onto a CD or DVD.
Step 3: Print and sign the following documents. We will need each of these four documents signed before we can accept your literacy narrative by mail:
- a Consent Form [Adult Consent | Minor Consent]
- a Release Form [Adult Release | Minor Release]
- a Deed of Gift or a License Form [Adult Gift | Minor Gift] ...OR....[Adult Creative Common License | Minor Creative Commons License]
- an About My Literacy Narrative Form |About My Literacy Narrative|
Step 4: Mail your CD/DVD and signed forms to the following address:
